IDENTIFICATION REQUIREMENT


Each examinee must present one form of identification bearing the examinee's name, photograph and signature. The name on the identification must be the same as the examinee's name on the roster. The photograph on the identification must look like the person presenting it. If you do not resemble the photo, you will not be admitted.

You must bring the following and present to the exam proctor for verification:
(1) Certificate verifying prelicensing course (Certificates are valid for one year from date issued.)
(2) Photo identification

Examples of acceptable identification are:
- driver's license
- passport
- military identification
- employee ID card

Examples of unacceptable identification are:
- social security card
- birth certificate
- credit card, with or without photograph
